How do I return my Brain Gauge once my subscription has ended?

If you'd like to cancel your Brain Gauge Home subscription, please fill out this form. 

Your Brain Gauge Home device must be returned in good/working condition to Cortical Metrics LLC within 14 days from the end of the subscription period. Return shipping is at the expense and responsibility of the subscriber. Please address all returned devices to:

Returned devices must be in working condition and show no evidence of damage caused by accident or abuse.We recommend purchasing insurance and using a shipping service that provides tracking updates. If we have not received your device within 30 days of the end of your subscription period, you will be charged a $99 hardware replacement fee. You are fully responsible for any loss or damage to the product during return shipping.
